What to Do with Creatinine 2.9, Hemoglobin 8, Protein 1+ and IgA Nephropathy

Question: I am an IgA Nephropathy patient for last 3 years. My creatinine level is 2.9, hemoglobin is 8 and protein is 1+. What measures I should take to stop my kidney failure in future.
